It's never easy.

Simple is best. Tell your students that you haven't been well and now the doctors told you that you couldn't continue teaching. When I took a medical leave, I told my kids (grade5) that I needed to go to the hospital for awhile and the doctors said I couldn't wait or I'd be very sick.As long as they know it isn't about them, and you wait until you only have a day or two left, it should be fine. It is also helpful if there is a day or 2 when you and new teacher can transition. The term "personal health issues "should say it all. It isn't anyone's business. I wish you well.:)
