Here's what I do

I print the double-sided version. It prints the pages in order so that you can then copy them back to back. You may have to try it once or twice to get them facing the right way because different copiers work differently. Then I use my long-arm stapler to staple them in the middle, and then I fold them.

There is also a conversion chart page that shows the equivalent levels to other reading programs. I don't know offhand what the tab says but if you have trouble finding it, let me know and I'll look it up.
