"homework for parents" sheet
By Gr2_girl

Clip to ScrapBook
Here are the questions that are on my Homework For Parents sheet. (I can't take credit as I got this idea from someone else.)

1. How does your child currently feel about school?

2. How often do you and your
child discuss school, and what do you discuss?

3. What school subject does your child enjoy the most?

4. What are your feelings about your child's homework assignments?

5. Where and when does your child usually do his/her homework? And what kind of homework assistance do you give?

6. What, if any, schoolwork has your child expressed concerns about?

7.
What concerns, if any, do you have about your child's interactions with teachers and/or other students?

8. In what ways can I help you and your child?

9. What questions would you like to have answered at our conference?

I really like these forms. It gives parents a voice, and helps to prepare you for any "unknown" problems that may arise at your parent conference.
As well, any problem that arises from this sheet may be well on the way to being corrected by the time the conference happens. (I send these out so they are due a week before the conference)

Hope you are successful finding a form that suits your specific needs. : )
