Basics from my form. Name of student, date all in attendance: Teacher concerns (I do this ahead of time) Parent concerns (I write as we talk) Plan of action (I write as we talk) I fill one of these out at EVERY conference and file it in the kid's folder in my room. That way i know what I said i would do...and waht we were worried about and I dont forget abotu the 2nd kid of the day when I had 14 conferences that day. :)
