I have a checklist with results from assessing the following: color words, vocabulary words, vowel/consonant sounds, running record scores ( at least 2), also, any skill in math that we've worked on. I always put the number right over the total number. Then by the time I get to the grade card, they've seen the scores and understand the grade given ( We give O, S+, S S- and U. Oh, I also check if they can tie their shoes, know address and phone number, and behavior. I give them a copy and keep one for myself to put in their homeroom folder for the next teacher to view.
