It sounds like you have everything covered, but I wanted to mention a book also. I subbed some, and I always appreciated having a chapter book handy that I could read to the class if I ran into 5 minutes here or there with nothing to do. I brought my own sometimes, but not all subs think of that.

I think someone mentioned emergency lesson plans - I keep those in my sub folder. If I'm absent, and can't get the real lesson plans there, the sub has something to go by. I have things like worksheets, video, newspaper activity (old newspapers in a cabinet in my room), etc...
