I made mine through microsoft word and I do a lesson plan page for each day of the week. I put it into a binder. I have sections for each day of the week and then I have sections for each subject. In the subject sections I put our monthly planning calendar as well as the state guidelines. I don't know how to put an example on here, but I can email it to you.
