I laminate a file folder and then on the inside I divide it into two. On both sides I put 3 x 5 file cards with each child's name printed on the top left hand corner...I leave a little room under each one so I can add more cards as the year progresses. On the left side I put notes about the child...and these aren't just negative or behavioral notes but anything that happens that I might want to remember later (i.e. report card time). On the right side I list when ever I have to make a phone call home to parents and a short note about the reason for the call.

I carry the folder with me all day long. It only takes a minute to jot down the basics of what's going on and at the end of the day I add to it if I need to.
