I subbed before, so I tried to remember everything I wish I had back then:
Class roll
Class schedule
Emergency lesson plans
2 reliable student's names (1 boy and 1 girl)
reliable teachers to ask for help
blank paper for the sub to leave notes on how his/her day went

I used a form that Mailbox Superbook has in it - it's wonderful to make sure you have all the names of the office personnel, etc... handy for the sub.
