I used to use a form for parent conferences, but when I went ot pull it out Friday I could not find it. I am still searching for it and if I run across it I will post it. Here is what the form had on it though: It had a place

for the child's name, my name, and person(s) attending the conference. It was a 5 column chart. The first column was where we identified problems, the 2nd column was where we set goals for the child, the 3rd was what I would do to help the child reach the goal, 4th was what the child would do, and the 5th was what the parent would do to reach the goal. Everyone would sign the form and

I would make copies to send home with the parent. This form along with work samples and test data and I feel prepared. I hope this is helpful. Again I will continue searching.
