I've taught 6th and 7th grades in the past, but this year I'm teaching fourth. This has been my policy in all my classes.

Under normal circumstances, I do not let a student retake a quiz/test. If a student fails a test (tests only), I will let him/her correct the test to earn back 1/2 point for every point s/he corrects. That usually keeps the student from failing the test, but it's still a low grade.

There have been times when my whole class bombed a test, and I allowed everyone to correct his/her test to earn back some points. I haven't had someone do poorly on a test due to death, unusual living conditions, etc., but if that happened I would make allowances if necessary.
