Hi There,
It may be an idea to set him up with his own 'procedure log' which tells him:

What he has to do
When he has to do it
Where he has to do it (e.g. write his homework in his homework book)
What he
has to do when he finishesFor example:
1. I sit down at my desk at 9 a.m. and take out my homework log (include photo of homework log)
2. I turn to today's page/this week's page
3. I write down the homework under today's day/date
4. I show it to my teacher for checking

Another thing that could work is having a buddy sitting with him who he could model off.

It
is really important that you give him pictures as well as words - like a procedural text of what to do and how to carry it out.

I would also give him his own daily timetable, that can be changed to suit the day. For example: 9 am Maths, 10 am reading. The words 'maths', 'reading' etc. are posted onto velcro dots that can be removed from the timetable stuck to his desk once they are finished.
I would also use praise for the kid when he does something right. Make it known to the whole class when he does something correctly!
